Why does Guru Meditation Mode happen?

I tried loading a Windows Vista VM, and Guru Meditation mode happened. I want to avoid this problem in the future. Can anyone tell me how this can be avoided in the future?
Moved to Windows Guests.
Post a VM log file. With the VM fully shut down, right click it in GUI. Select "Show Log" and save "VBox.log" (ONLY) to a zip file. Attach the zip here.
