Yes. You can make guests almost anywhere your PC can store data, with varying degrees of speed. It is possible to put a guest on an "external" hard drive (I think that's what you mean by "extended"). Set your new guest location in the Virtualbox window's File menu, Preferences, General, Default Machine folder. All new guests will go in that designated folder. Old guests will remain where they are now. If you want to move an old guest, see Moving a VM.

A regular USB2 drive will have slower disk access speeds, therefore a slower guest. USB3 will have closer to "internal drive" access speeds.

There can be problems on Windows hosts changing the external drive's drive letter, causing your guest to be inaccessible until you change the drive letter back to the correct letter. Having your drive's letter up toward the Z end of the alphabet instead of down towards the C end will help to keep the drive letter from changing. Google for how to change a drive letter in Windows, to put the drive letter higher in the alphabet or in case the letter ever does change.

If you really want to murder a guest stored on that external drive, unplug the drive while the guest is running. (In other words, don't unplug the drive while the guests are running...)

It would be better if possible to put your guests on a second internal drive, but a USB drive will work.
