Just installed VB 4.3.22 with a Windows 7 VM and went to install the extension pack 4.3.22 and got the following error 'failed to open the extension pack'. The details of the error are

RTVfsFsStrmNext failed: VERR_TAR_BAD_CHKSUM_FIELD

Tried downloading several times and made sure extension pack was 4.3.22 to match VB version. I ran a disk verify and had no issues.

Any ideas?

That same installation worked for me, although subsequent problems with 4.3.22 Guest Additions for Windows caused me to drop everything back to 4.3.20.

AFAIK, Disk Utility's "Verify Disk" function ensures that the file system is correct; it does not attempt a low-level verification of the byte contents of every sector. If you have an undetected bad spot (admittedly a long shot), you might want to rename the .vbox-extpack file (so that the disk space is still allocated), and then download again (into what will then have to be a different space on your HD).

Next step would be to try the install on an external drive, and the step after that would be to try on a different Macintosh computer.

and compare it with the publicized ones in https://www.virtualbox.org/wiki/Downloads
To make your life even easier, type "md5", <space>, and drag and drop the extension file to the terminal.
