adaptor is set to use the usb adaptor:

networking macos side is connected and working through this adaptor only with others disabled

Windows-side (in the vm) I can't get networking going properly. The machine is set to receive info from DHCP (fixed IP assigned by mac address, of course)

If I enable wifi on the mac and connect to the same LAN, then change the network adaptor on the VM to bridge to the wifi adaptor:

then the networking on the VM works again (without re-binding anything - I can just ping again). In the log below it first times out (via USB adaptor), then it starts working (via Wifi)

I have now upgraded to VirtualBox 4.3.14 r95030 and also downloaded and updated the additions, and reapplied them to the virtual machine and rebooted it. I can confirm that this has had no effect on the problem.
