Installing Guest Additions for 4.3.10.R93012

I am trying to install the guest additions for Version 4.3.10r93012 as recommended by the pop-up window which appears after I start Windows 7 Professional (Service Pack 1). I am using Virtual Box on a 2009 Mac Book Pro running OS X version 10.9.2. My current guest additions are listed as 4.3.8r92456.

I am assuming the following: the virtual machine = Virtual Box; The host is my MacBook Pro / OS X, v10.9.2; the guest is Windows 7 Professional.

I am following these instructions from the manual...
4.2.1.1. Installation

In the "Devices" menu in the virtual machine's menu bar, VirtualBox has a handy menu item named "Insert Guest Additions CD image", which mounts the Guest Additions ISO file inside your virtual machine. A Windows guest should then automatically start the Guest Additions installer, which installs the Guest Additions into your Windows guest. Other guest operating systems (or if automatic start of software on CD is disabled) need manual start of the installer.
If you prefer to mount the additions manually, you can perform the following steps:
1.Select "Mount CD/DVD-ROM" from the "Devices" menu in the virtual machine's menu bar and then "CD/DVD-ROM image". This brings up the Virtual Media Manager described in Section 5.3, “The Virtual Media Manager”.
2. Start the virtual machine in which you have installed Windows.

My problem:
1. The guest additions do not automatically install. I have shut down, restarted and re-tried four times. ( I am reminded of the definition of insanity)
2. In attempting the manual approach, step 1 which arises one to select "Mount CD/DVD-ROM" from the "Devices" menu ... this does not exist in my "Devices" menu nor any of its sub-menu items
3. Assuming Host is my MacBookPro / OS X I am at a loss as to what (Host + D) means. I have researched it on the web and the best I can find that it refers to the 'Right Control' key ... my MacBook and iMacs do not have such a key.

Can anyone help me with this issue... also are the guest additions involved with my inability to use any USB ports?? Do these additions allow me to access my documents folders in my MacBook Pro??

Guest additions never automatically install in a Win7 guest, because Win7 disables autorun. No version of VirtualBox changes Win7's behaviour.

With Win7 running, choose Device|Insert Guest Additions CD. Browse to the CD inside the guest and run the VBoxWindowsAdditions.exe file. Remember to unmount the GAs CD afterwards.
