No USB devices for Linux Guest to grab

I'm running VB4.3.6 on Win7, with a Debian guest (although another Puppy Linux guest has the same problem).
When I connect a USB device, it shows up on the Host, but not in the Device list for the guest to grab (there is a filter from the previous VB version where it worked), it just says 'X No USB Devices Connected'
In the Host USB Device list it's listed as Virtualbox USB/<VolumeID>, but 'vboxmanage list usbhost' says:

Did you install the Extension Pack?

I'm running the same Host and VB versions, except my Guest is Ubuntu 12.04 or 12.10. Both of those get newly plugged-in USB devices in the Devices menu. If I want to make the Guest auto-attach to the device, I add a USB Device Filter in Machine->Settings->USB. After that the device always shows up in the Guest when it is plugged-in. I ran VBoxLinuxAdditions.run in Linux to add the Guest Additions. This executable is on the CD image in VBoxGuestAdditions.iso, which is stored in the same directory where VirtualBox.exe runs from.

Perhaps:

- the ISO you're mounting within the VM is not the one from the current version of VirtualBox.
- you didn't install GuestAdditons in this particular VM.
- Debian upgraded the kernel, and you didn't re-run the GuestAdditions installer since then.
- the VirtualBox installation need to be repaired with its Windows installer.
