This is an informational post rather than a request for help.
I was getting a consistent and often apc_index_mismatch blue screen under Windows 7 (64 bit). The dumps showed a different driver as responsible each time.
It took me a lot of trial and error to discover that the problem is in VboxNetAdp.sys. So, if you are getting apc_index mismatch BSODs, I'd suggest uninstalling VboxNetAdp.sys. Remember to remove the references in the registry to the driver as well.
VirtualBox runs fine for me without VboxNetAdp.sys (including all networking functions), so I'm not even sure what I'm missing by uninstalling it (besides BSODs <grin>).
