In some cases (SCSI HBAs), the geometry specified in the image will definitely be ignored. With IDE, it shouldn't be. Why not attach the VBox.log file which would unambiguously show what's really going on?
As others have requested in related reports at the bug-tracking database, it would be great if VBox would allow advanced users to manually specify CHS disk geometry.
We accept patches :)
