lost audio after having connected via MS Terminal Client

host: Win 7 x64
guest: lubuntu 12.10 (but this was happening on any previous versions too)
virtualbox tools on client: installed
add-on: previously not installed, now installed
virtualbox version: 4.2.8 (but this was the same for the last few years)
Symptoms:
I connect from one PC to my host Windows over RDP (Windows Terminal Services) with audio redirected to the connecting client.
After disconnecting and re-using the Windows host with Linux guest, the Linux guest can't use audio; the errors are:
pulseaudio[2168]: [pulseaudio] sink-input.c: Failed to create sink input: too many inputs per sink.
I have tried to restart pulse and alsa:
pulseaudio -k && sudo alsa force-reload
Result: when I try to play an audio file, audacious opens the playback but it freezes at the beginning and doesn't progress (like if it started paused); no log entries found for that...
