I have a Win7 64-bit host and it wants to do an automatic Windows update. Do I need to close the VM and save state before doing the update? Also, in everyday operation, can I hibernate the host without shutting down the guest?

Windows should handle this all automatically, i.e. if it is updating some stuff still being used by VBox, you'll have to restart after the update. No harm in saving the state of your VM and exit VBox of course, just to be on the safe side.
I regularly sleep/hibernate my Windows7 host with VBox still running. I haven't had any trouble with that in a long time. Again, safest option is of course to save your VM.

I also let Windows do its updates while VMs are running, without trouble to the VMs. I do the automatic-download-manual-start Windows Update, so I don't get taken by surprise. I will shut down or save-state the VMs before letting the host restart, and I try to have a script that save-states the VMs if the host back-up battery orders a hibernation. But I have had a host without the save-state-the-VMs script hibernate due to power loss while a VM was running and the VM picked up just fine when the host restarted.
