Hey,

I get this error message when trying to install (after entering my Mac OS password):
Failed to install the Extension Pack /Users/aheinon/Downloads/Oracle_VM_VirtualBox_Extension_Pack-4.2.0-80737.vbox-extpack.

Referenced from: /Applications/VirtualBox.app/Contents/MacOS/ExtensionPacks/Oracle_VM_VirtualBox_Extension_Pack/darwin.amd64/VBoxPuelMain.dylib

Reason: Incompatible library version: VBoxPuelMain.dylib requires version 4.2.0 or later, but VBoxVMM.dylib provides version 4.1.22.

Result Code: NS_ERROR_FAILURE (0x80004005)
Component: ExtPackManager
Interface: IExtPackManager {3295e6ce-b051-47b2-9514-2c588bfe7554}
Was something missed in the update?

It's almost like you're trying to install the 4.2 extension pack but are still running 4.1.22 VirtualBox.

I had the exact same issue. Turns out the update to 4.2 somehow DOES NOT overwrite the 4.1 application, and when you start VirtualBox from Spotlight 4.1 gets started again. I had to do a full uninstall and purge everything, and check under File -> About VirtualBox to make sure the right version of VirtualBox was running.

I hate to repeat myself but I had no issues updating to 4.2.0 or installing the ext pk. running OSX 10.8.1

Yep, that was the issue. I reinstalled version 4.2 and now the extension pack installs just fine. Weird.
