File open dialogs broken (e.g. choose OS disk image)

I have just installed VirtualBox 4.1.18 for the first time on OS X 10.7.4 (on a MacBook Pro with Retina Display) and am having a serious issue creating new VMs. VirtualBox’s File Open dialogs seem to be broken.
For example, in the ‘Create New Virtual Disk’ wizard, at the step where I am prompted for ‘Virtual disk file location and size’, clicking the browse button for the ‘Location’ initially opens a native Save As panel, which after a few moments is replaced with VirtualBox’s custom ‘Select a file for the new hard disk image file’ window. In this window, I am unable to navigate to any path other than the initial selection (my home directory).
Selecting entries in the ‘Look in’ drop-down menu does not cause the file listing to update for the newly-selected folder. Selecting locations (Computer, or my home directory) from the left-hand pane also does not update either the file listing or the ‘Look in’ drop-down menu. Double-clicking folders in the file listing and clicking the ‘Parent Directory’ button likewise doesn’t update the file listing or the ‘Look in’ drop-down menu.
Fortunately in the case of the ‘Create New Virtual Disk’ wizard, we are free to type the location for the virtual disk file by hand. The same is not true of the ‘Media Source’ field in the ‘First Run Wizard’, which opens a ‘Choose a virtual CD/DVD disk file’ dialog that suffers from the same issue. Because this dialog is non-functional, and there is no alternative means of specifying the disk image location, I am unable to boot from a disk image on first boot in order to install my operating system.
