I'm curious. I look in my console, and I see the following, as I start up Mac OS X Snow Leopard.

6/15/12 4:03:26 PM com.apple.SystemStarter[53] /Applications/VirtualBox.app/Contents/MacOS/VBoxHeadless => /Applications/VirtualBox.app/Contents/MacOS/VBoxHeadless-x86

6/15/12 4:03:26 PM com.apple.SystemStarter[53] /Applications/VirtualBox.app/Contents/MacOS/VBoxNetAdpCtl => /Applications/VirtualBox.app/Contents/MacOS/VBoxNetAdpCtl-x86
6/15/12 4:03:26 PM com.apple.SystemStarter[53] /Applications/VirtualBox.app/Contents/MacOS/VBoxNetDHCP => /Applications/VirtualBox.app/Contents/MacOS/VBoxNetDHCP-x86

6/15/12 4:03:26 PM com.apple.SystemStarter[53] /Applications/VirtualBox.app/Contents/MacOS/VBoxXPCOMIPCD => /Applications/VirtualBox.app/Contents/MacOS/VBoxXPCOMIPCD-x86

6/15/12 4:03:26 PM com.apple.SystemStarter[53] /Applications/VirtualBox.app/Contents/MacOS/VirtualBox => /Applications/VirtualBox.app/Contents/MacOS/VirtualBox-x86
6/15/12 4:03:26 PM com.apple.SystemStarter[53] /Applications/VirtualBox.app/Contents/MacOS/VirtualBoxVM => /Applications/VirtualBox.app/Contents/MacOS/VirtualBoxVM-x86
6/15/12 4:03:26 PM com.apple.SystemStarter[53] /Applications/VirtualBox.app/Contents/MacOS/vboxwebsrv => /Applications/VirtualBox.app/Contents/MacOS/vboxwebsrv-x86
6/15/12 4:03:26 PM com.apple.SystemStarter[53] Loading VBoxDrv.kext
6/15/12 4:03:26 PM com.apple.SystemStarter[53] Loading VBoxUSB.kext
6/15/12 4:03:27 PM com.apple.SystemStarter[53] Loading VBoxNetFlt.kext
6/15/12 4:03:28 PM com.apple.SystemStarter[53] Loading VBoxNetAdp.kext

To the best of my knowledge, I don't have anything related to Virtual Box in my startup items. Why is this showing up every time I boot the Mac?

"Startup Items" relates to what you want running when you log into your account. The activity you see related to VBox is what runs when the system boots. It is possible to have VMs running at boot time, that are accessible, without a user having logged onto the host. I have been using vboxtool and phpvirtualbox on Linux hosts to manage headless VM operation, but they should work as well on a MacOSX host. (probably better -- OpenSuSE has some serious issues with not installing the default developer configuration, giving problems with both host and guest.)
