I am using 4.1.12 on an XP pro system, with an XP Pro VM. I've been using the VM machine for months. I often have it up and running for days.
Last week I went to shut it down, and it hangs with a progress bar stuck at 42%. I had this happen a month or two ago and I killed the process. Now that VM is "inaccessible". A) why is this happening? and B) is there a way to recover an "inaccessible" machine?
I still have the progress bar up in case there is some way to salvage this.

Not exactly no. There have been a few where the guest gets all the way down and hangs but stopping the guest at that point did not kill the guest
I would report this to bugtracker. If you have not signed on to Bugtracker before you will need to select a new Nickname to be able to report issues.
It would also help if you can post the ticket number here so other can see the progress, add information, or see the final results.
