I'm told I need to re-install Guest Additions from safe mode to enable
D3D, but when I click on "Devices" -> "Install Guest Additions" while in
safe-mode nothing at all happens.

I am running Windows 7 32bit guest under VirtualBox 4.0.4
on Kubuntu 10.10 32bit.

Autorun probably fails in safe mode. Go to the CD/DCD icon in Computer and run VBoxWindowsAdditions-x86.
