I made a vm of redhat 2.4.21 and usb will not work. When the vm boots I do get a usb-ohc failed to load. This happens on both a Windows 7 and OSX machines.

I have added the extension pack to both machines.

redhat 2.4.21 is the kernel not the version but from the looks of it you are using a very old version of Redhat. This may be the issue. Not certain though.
