Many rudimentary things broken in VBox 4.0.2

Not sure where to even start, but here's a list of things that are not working with Linux guests:

- send ctrl-alt-del to host does absolutely nothing
- Device->Install Guest Additions - no disc shows up, not on the desktop, not under /media - nowhere. I have to log out and log back in to see the disc, or sometimes reboot.
- update checker is broken, I had 4.0 for the longest time and even if I did "check for updates" it said I was on the latest, even though 4.0.2 is out

Most alarming is #2. If I try to mount an ISO, it does NOT show up. Can someone explain why this is happening? My guest is Mint 10.
