Upgrading my guest additionals, in my 64-bit W7 HPE SP1 guest VM, cause failed reboots.
It keeps going to Startup Repair (https://prnt.sc/vKUWLUaWYNVR for a screen shot/capture since I can't attach in my forum post since its file size was too big). I even tried in safe mode to upgrade drivers. I see this in both of my updated 64-bit W10 Pro and macOS Mojave v10.14.6 hosts after updating VBs (v6.1.48 in Mac & v7.0.12 in 64-bit W10 Pro)). Is anyone else seeing this too on their end?

I have exactly the same problem with 6.1.48 on Windows 7 Ultimate 64-bit. Am back to 6.1.46

Is there a bug report of it?

The last working Virtualbox for a Windows 7 host was VirtualBox-6.1.45-156947-Win.exe.
All 6.1.47 and later releases are broken for both Windows 7 and 8.1 as a guest and host. Probably the same for 7.0.11 and up. Might need testsigning.

Windows 10-64 host, Windows 7-64 guest, reboot after update GA 6.1.48 started Windows boot repair,
then login to Win 7: GA 6.1.46 are running.
Tomorrow, I will try to download and import the missing root certificates in Win 7 before I update GA to 6.1.48 again
as this may resolve the GA 6.1.48 installation errors.

Exact same scenario happened to me today.
Win11 host, Win 8.1 guest, upgraded from 7.0.10 to 7.0.12
Any way to recover?

This is a problem with the Guest Additions. Please see the 7.0.12 discussion topic.
