At some point you tried to add media to a CD drive. Instead you added a second CD drive. It's quite easy to do, I've even done it myself in the dim and distant past before I worked out what the cryptic icons on the <settings>|Storage dialog all did, though in my case I noticed immediately and fixed the problem right away.
