I'm hitting this when booting cisco IOSv in virtualbox 4.3.26 r98988 (release build):

According to the Cisco site, it should run in VirtualBox 4.2+. I looked at your log. you only have 2GB of available memory out of 16GB. What are you running?

TRIPLE_FAULT usually seems to mean that the guest OS is doing something real nasty. VT-x systems will be more tolerant of that: I would try enabling it.

Otherwise, and especially with a readymade image, you need to make sure that the VM recipe matches the expectations of the guest OS. If they supply a working VM then use it - don't try to build your own VM around the disk.

There is supposed to be an .ova with the proper settings. It does require 4GBs and 2 virtual CPUs. With the OP's log indicating there is only 2GB available that could be an issue.

The user log indicates only 512MB RAM is assigned along with 1 CPU.

I'm booting vIOS, not VIRL, so the memory requirements are very low.

I turned off VT-X because it's crashing my host (macosx kernel crash) when it's on...

It works fine with QEMU.

Should I open a bug in the bug tracker?

This should be fixed in the upcoming VirtualBox maintenance release for 4.3.x. The invalid opcode was caused due to an incorrectly cleared CR4.VMXE and VMXON execution which was caused due to a fix done earlier for Mac OS X Yosemite where we fellback to manually initializing/de-initalizing VT-x as we failed to find certain kernel symbols because some Yosemite update moved the file from /mach_kernel to /System/Library/Kernels/kernel.
