Re: 4.1.2 Failure to Load
From what I read here you need to run the Repair Disk Permissions using Disk Utility on your MAC host then it should work.

Re: 4.1.2 Failure to Load
Repairing file/disk permissions didn't help me. Error persists. Have also uninstalled/re-installed VBox to no avail. Any other ideas?

Re: 4.1.2 Failure to Load
I found the fix:
However, I think I know how this happened... I got the computer from a friend who had an admin profile, but also had sensitive information on the hard drive. The simplest fix to make sure all of her personal things were cleaned up was to create a 2nd admin account for me, then delete her profile (and all associated files). The problem is that apparently back when the 10.3 system was installed, the root permissions were tied to her specific profile, etc, etc, etc. So changing the perms on /Applications folder now seems to have finally erased her from the computer's mind forever. Let's hope that Apple continues to make their OSs more friendly/intelligent rather than following in Micro$oft's footsteps.
It came from https://www.virtualbox.org/ticket/78892010-12-27 03:05:00 changed by vadiml ¶
Finally after examining Virtual Box log files i saw that it complains that /Applications folder is not owned by root... a "sudo chown root /Applications" fixes the problem

Re: 4.1.2 Failure to Load
In my case, it was the installer of the drivers of the samsung ml2570 printer that screwed the permissions of /usr and /usr/lib in linux. notice this is being tracked by vb to see if they can skip the ownership requirement: https://www.virtualbox.org/ticket/7889
