I am having the same issue. I get similar output that kahun gets. Just gid number different,etc,but same error code. I'm running gentoo . 4.0.4 worked fine.

It's not limited to Natty. I'm having this same issue on Lucid (Ubuntu 10.04) and Natty (11.04). 4.0.4 was working on Lucid before updating.

Edit: Adding the group "vboxusers" to my account solved the problem.

Archlinux also... Must have been a change that was missed...

I am having the same problem with Debian Squeeze (fresh install). I tried everything here, but nothing solved the problem.

I have the same problem.
Partial solution is to run the VirtualBox under privileged account root.
This works for me...

Sorry, I forgot to say that.

I'm using VirtualBox 4.0.6 in a Debian Squeeze (6.0) fresh install.
When I install the extension pack, everytime that I enter the machine's config, I get that error: "Failed to access the USB subsystem" - NS_ERROR_FAILURE (0x00004005).

Can anyone help me?

Search some threads back. There was a suggestion, that the user must be member of vboxusers and a group called like the user..

After scouring the forums I had the exact same error on a fresh install of Ubuntu 11.04 Natty and Virtualbox 4.0.6. I ran "sudo /etc/init.d/vboxdrv setup" (without quotes) and this worked for me.
I was able to access my USB devices too in the virtualbox. The guest OS is Windows XP.

anyone found a fix to this ? I havent even upgraded to Naty yet, still on Maverick and mine is broken as well! I tried running the suggested code here is what i got for an output (it did not fix problem for me )

Check to see of you have a package installed called "dkms", if not, then install it and try running VB, or you may have to also reinstall VB. The above link also states "I have seen where dkms had become corrupt, but that has been a long time ago. Usually a remove purge of dkms and then installing it again fixes this."

So if dkms is installed, try doing a remove/purge of that package: sudo apt-get remove --purge dkms, reinstall dkms, then try running VB.

You may try to make a virtual hd to SCSI controller, not ide controller
I also have same problem too few days ago

I also post it in this forum few mins ago

This problem started for myself when upgrading to 4.0.8 under Ubuntu 11.04 64bit (guest is Win7) but this error message appeared before I launched a guest. Tried many of the suggestion in this thread with no success. Finally got a working system by downgrading to 4.0.6

I was seeing this error when entering a wrong administrative/root password. I entered my normal user password by mistake, as I usually do whith a "sudo su".

One way to also solve the problem is to install the extension pack from the shell:
# sudo VBoxManage extpack install Oracle_VM_VirtualBox_Extension_Pack-4.1.12-77245.vbox-extpack

I hope this helps.
