I have/had the same problem. I run VirtualBox 3.0.12 r54655 on Ubuntu 9.10 and kernel 2.6.31-15-generic #50-Ubuntu SMP Tue Nov 10 14:53:52 UTC 2009 x86_64 GNU/Linux.
First I had problems with a CentOS 5.4 guest. The time was running at about half speed. The trick with the rt kernel solved this problem. With every new kernel I checked whether the problem has been solved and one or two weeks ago it finally was.
I never had problems with a Windows XP guest until today. I don't know whether it appeared with the last VirtualBox update or with the latest guest additions I installed today. The time was running sometimes too fast, sometimes too slow. After trying several things I seem to have solved the problem by disabling the VirtualBox Guest Addition Service.
Maybe you or anybody else with the same problem want to try this as a workaround for now and sync your time in another way.

I have VBox 3.0.12 on Kubuntu 9.10 x64 running Win XP 32-bit guest. Same problem with multiple vCPU. See bug http://www.virtualbox.org/ticket/5501

Could you check if applying the change as posted in the ticket 5501 improves the behavior?

So, the solution to this is to install a kernel from the Lucid PPA - am I understanding that right? If so, what's the link for installing the Lucid PPA?

Same problem here. Solved today by decreasing the number of processors from 2 to 1.
VBox: 3.2.8
Host: Ubuntu 9.10 64 bit, AMD quad core, 8gb ram

I am afraid that you have not solved the problem. Reducing the CPU to a single processor always worked. The only way to fix it is to install the newer kernel from the PPA as described in the trac. I see that Ubuntu has also tried to fix the default 9.10 kernel but have not tried that fix.
