The crash is "Triple fault", meaning the guest did something nasty. Perhaps the floppy image is corrupted. Where did you get it?

Actually IBM OS/2 1.0 and 1.1 is known for triple faulting intentionally (286 style protected -> real mode switching). Which is why the OS/2 1.x guest type in VirtualBox enables CPU reset on triple fault.
