Post the complete guest log file. Also it would help to know the bit of the guest ( 32/64 )

I ran into the same issue while trying to pxe boot cent 5 in a VM from an external pxe server. The full log is attached. The host is OSX 10.11.6, client is intended to be a 64bit machine, no OS is actually installed yet as PXE never got beyond this point.

Edit: Virtualbox version is: Version 5.1.4 r110228 (Qt5.5.1)

Further edit, reverting back to Virtualbox Version 5.0.26 r108824 fixed it completely.

This issue is also present on VirtualBox versions 5.1.6 and 5.1.8 with a CentOS 5 guest (at least with both vagrant boxes I was attempting to use) and a macOS 10.11.6 host.

I will try installing the most recent 5.0 release (5.0.28-111378) and see if that works. EDIT: Yes, it works. So it seems 5.1 has issues not present in 5.0. I'll stick with 5.0 until/unless I hear this is fixed. :)

@Weildcard
Have you tried it standalone, i.e. without Vagrant?

I assume this is CentOS 5.8 or so. This is a known problem that happens only on OS X hosts and only with specific old Red Hat/CentOS kernel versions. 5.7 and 5.8 are known to be affected. These are classic Red Hat frankenkernels which claim to be 2.6.18 but look nothing like mainline 2.6.18 kernel.

The bug was fixed right after the 5.1.8 release. It may be possible to work around it by turning off KVM paravirtualization.

I just upgraded to 5.1 and had the same issue on macOS Sierra (10.12.1) and VBox 5.1.10

I was unable to boot VMs I had running for several months.

Removed KVM paravirt and problem disappeared.

Thks :D :D

Should be finally fixed with the most recent (>=112141) 5.1 test build for Mac OS X. This fix will be part of the next 5.1.x maintenance release.
