Guest losing disks?

Good evening,

Host system: HP DL380 Gen9 series, 16 cores, 64GB RAM, 4.19.0-14-amd64 #1 SMP Debian 4.19.181-1 (2021-03-19) x86_64 GNU/Linux

I have a situation where a virtual machine has become unresponsive with the following error message repeated on the console:
"EXT4-fs error (device sda1): __ext4_find_entry:1449" (also "device sdc1" is affected, and probably sdb1 as well).

The machine wouldn't stop on "controlvm", but the stop progress hung at 20%, never reaching 30. I had to manually kill the process. On restart the VM came up instantly (checking the file systems, though). At the time of the hang, the option "Use Host I/O Cache" was disabled (in another thread where a vm got stuck, someone suggested enabling it).

The log file is attached. I wonder if someone could help me shed some light on this?
