I couldn’t figure out why adding a virtual machine fails and ends up with “UUID already exist” error
What could be the issue here?
How this issue can be resolved?

You can't register the same VM twice. If you want a copy of the VM, clone it.
If the software is complaining because of the ISO, then use the Virtual Media Manager to release the ISO from all existing VMs before adding the new VM.

But how come they are the same?
As per the screenshot below, I have two: one vbox for windows 11 and the other is for windows server 2022.
Why their UUI is considered the same as the are added to the vbox?

The error says there are two items with the same name. But, get ready for this, it's not the VMs that are indicated as the items with the same name. What does the error message say are the items with the same name? Once you wrap your mind around that, then you'll see why Mpack suggested to
